It was a flaw hidden within the central stellar ring of the Magic Rune. A subtle difference in angle created an extra channel between the intersecting lines of two corners.
Most of the time, this channel didn’t cause any problems. But if a Caster’s Spirit isn’t focused enough during the drawing process, that line will register the Rune as flawed, directly causing the Magic to fail.
"I’ll answer two questions. One, not all Magic Runes are flawed. At least from the Magic Runes I’ve learned so far, some are without defects. Two, fixing the flaw doesn’t mean failure is impossible. After all, not everyone can draw a perfect Rune every time."

"If you express opinions so carelessly, without even attempting to verify the truth," Wei’en said coldly as he descended the altar, "then I am... very disappointed in you Wizards of the Spiritual Mystery School!"

In fact, the real reason he had chosen to reveal the Rune’s vulnerability was to test a theory he had—whether only the Magic Rune knowledge obtained from the Ghost Soul School was flawed.
