"When did you get this scar on your back?"
Xu Yino asked the patient.
"What scar? What’s a scar?" the patient asked.
"Hold on, I’ll show you. There are two marks, one is oval-shaped, about one centimeter. The other is elongated, about two centimeters," Xu Yino said.
"The circular scar also has an ulcer. Don’t you feel any pain? Have you scratched it with your hand? No, you probably can’t reach this position."
As she spoke, Xu Yino gently pressed the area of the scar on the patient.
"Crack, crack, crack..."
A series of fine, fragmented sounds emerged from the location of the scar as she pressed.
Upon seeing the scar, both Xu Yino and Liu Yiqing felt it wasn’t a good sign. Now, hearing that sound, they were startled.
It wasn’t the sound that startled them, but rather the new discovery in the patient’s condition. It had suddenly escalated from possibly just enteritis to a few levels higher.
"Nuonuo, stay here now, I’ll go call Teacher Liu, and notify the operating room." Liu Yiqing said as she ran out.
"Doctor, what’s happening to me? What’s wrong with my back?" the patient asked with a slight tremor in her voice.
"You need to be hospitalized. When our Teacher Liu arrives, he will inquire carefully about your condition over the past few days," Xu Yino said.
"Don’t worry, our Teacher Liu is very skilled. You just have an ulcer on your back now, and we need to do a debridement, anesthesia will be administered, it won’t hurt."
Right now, she could only comfort the patient this way. Although she had a diagnosis in mind, the issue was too severe and had to be addressed by Liu Banxia.
After finding Liu Banxia, Liu Yiqing, without saying a word, dragged his arm towards the treatment room.
"What are you doing?" Liu Banxia said helplessly.
"Teacher Liu, our patient seems to have gas gangrene on the back, and a cracking sound like an eggshell breaking could be heard upon pressing," Liu Yiqing reported.
"Quickly, prepare the operating room, and get ready to draw blood for a culture," Liu Banxia instructed.
He hadn’t anticipated that the patient two girls attended to would actually have gas gangrene, a very tricky and troublesome condition. Once gas gangrene occurs, it quickly induces toxemia in a very short time.
However, he was a bit puzzled as gas gangrene typically occurs in wounds caused by firearms and is one of the most serious and quickly developing complications in such wounds.
Unlike in movies where a person can survive multiple gunshots and explosive blasts. It’s not impossible, but that’s really a stroke of great luck.
In former battlefields, many died after being wounded without timely treatment and debridement, developing gas gangrene, which led to amputations or even death.
Such casualties accounted for a significant proportion of deaths in battles.
"What are the prodromal symptoms?"
Upon arriving at the treatment room, Liu Banxia asked.
"Abdominal pain, diarrhea, fever, slightly elevated white blood cell count, mild anemia, and mental tension," Xu Yino quickly replied.
Liu Banxia put on gloves and also pressed the patient’s scar. That "crack, crack," resembling an eggshell being crushed, was soft yet distinctly audible.
"No doubt about gas gangrene, proceed with admission, debridement, blood culture, fluid replacement, and adjust treatment against toxemia," Liu Banxia instructed.
"Understood,"
Xu Yino acknowledged and covered the patient’s back with the clothing, then with Liu Yiqing pushed her outwards.
"Doctor, doctor, what’s going on?" Upon seeing the scene, the patient’s boyfriend outside was frightened.
"Her acne on the back ruptured, leading to gangrene. We need to perform debridement and treat toxemia. Please get her admitted, then I’ll have more questions for you," Liu Banxia explained.
"Tongtong, Tongtong, are you alright?" the patient’s boyfriend asked her again.
"Did you notice anything wrong with my back when you scratched it yesterday?" the patient turned and asked.
"No, what’s really going on?" the boyfriend said.
"Go handle the procedures first; it seems quite serious," the patient said.
Liu Banxia signaled to Xu Yino that there was no time for them to bid a long farewell.
It’s a race against time for the patient since toxemia had already set in, affecting her mental state.
Yet, there’s a need to identify the specific bacteria causing it to provide targeted treatment. Even though he had an initial assessment, further verification was necessary.
Xu Yino and Liu Yiqing pushed the patient for preparation, while Liu Banxia followed the patient’s boyfriend to ensure the admission procedures were complete.
"Did you scratch his back yesterday?" Liu Banxia inquired.
The patient’s boyfriend nodded, "Doctor, did I scratch it? I felt like I scratched a pimple, but it shouldn’t have broken."
"It’s just a possibility right now. Has she suffered any other injuries recently, or come into contact with anything else at home?" Liu Banxia asked.
"I don’t think so. I just came back from my hometown yesterday. My mom killed a chicken, and I made mushroom stew for her. She loves stone pot bibimbap, so I made that too, and I already told the other doctor." The patient’s boyfriend replied.
"Okay, you wait outside and get in touch with her family. Her condition is developing quickly, and her family may need to help make some decisions." Liu Banxia said.
"You’re just the boyfriend, your authority isn’t enough. Contact them quickly and ask them to come. Even if we have the blood culture results, it’s uncertain how the medication will work."
After speaking, Liu Banxia ran toward the operating room.
The patient’s boyfriend was already petrified, as Liu Banxia’s implication was that even treatment might be dangerous.
"Teacher Liu, everything is ready."
After Liu Banxia entered the operating room, Xu Yino said.
Liu Banxia nodded, "The gangrene is on the back, you two proceed, and I’ll observe."
"Uh... okay." Xu Yino hesitated for a moment, then hurriedly began to prepare.
"This is really dangerous. Discuss what should be done next." Liu Banxia said.
"Doctor, am I dying?" The patient asked, lying on the operating table.
"You shouldn’t die, but it will be quite painful." Liu Banxia said.
"We need to see how large the area of gangrene is on your back. If it’s extensive, you might not be able to wear backless dresses in the future. We’ll see during debridement; just stay calm."
Xu Yino and Liu Yiqing exchanged a glance and caught the key point.
The Big Demon King definitely knows which bacterium is causing the infection, otherwise he wouldn’t be so sure. But what bacterium is it?
"You haven’t answered my question." Liu Banxia said.
"We need to collect samples for culture, which might produce results faster than the blood culture." Xu Yino said.
"And then?" Liu Banxia continued asking.
"Then... then it’s debridement, complete debridement. We must thoroughly check for other gangrene spots on the body, ensuring thorough debridement." Liu Yiqing added.
"And then? What about the choice of antibiotics?" Liu Banxia asked.
The two girls remained silent. Antibiotics can’t be used carelessly, and the patient’s condition is still in its early stages. Using the wrong one could harm the patient.
Given the patient’s current situation, only large doses will be effective.
"Doctor, there’s only the three of you, isn’t that too few people? Isn’t anyone else needed?" The patient lying down asked.
"It’s not abdominal surgery; you don’t need general anesthesia or spinal anesthesia, just debridement with local anesthesia." Liu Banxia said with a smile.
"But from your situation, your mind seems quite sound, keep it up. The development of the condition will surely make you uncomfortable for a while, you have to hang in there."
"Director Liu, should we do sensitivity testing now?"
The nurse nearby asked.
"You can." Liu Banxia nodded.
"Teacher Liu, what bacterial infection do you suspect?" Liu Yiqing asked.
"Can’t you use a bit of your wit? She doesn’t know what’s happening, and neither do you? You can also deduce from the conclusion. Have you ever seen gas gangrene before?" Liu Banxia replied.
"No, that’s why I’m at a loss." Xu Yino said frankly.
"Are you scared by our conversation? Don’t worry, I’m just teasing them. The incision is already made, and the gangrene doesn’t seem very large or deep, so there shouldn’t be any major issues. You can still safely wear a swimsuit." Liu Banxia reassured the patient.
The patient felt odd and complex, never having imagined experiencing such a scene. Initially scared to death, now hearing their conversation, it doesn’t seem like a big deal.
"Quiz question: What causes gas gangrene?" Liu Banxia continued.
"Well, there needs to be suitable bacteria, some necrotic tissue, and it must be in a relatively closed environment. Oh, it should be anaerobic bacteria, can it be treated in the hyperbaric oxygen chamber of the inpatient department?" Liu Yiqing said.
"That’s later on. My question is just to help you recall potential pathogenic bacteria for gas gangrene, a very simple question. It’s like asking what you had for lunch today, you say food, and after the meal you can take a bath." Liu Banxia said.
"Pfft..."
The patient couldn’t hold back and laughed, slightly affecting Liu Yiqing and Xu Yino’s actions.
"Alright, not nervous anymore, right? You still need to relax." Liu Banxia said.
The patient finally understood why Liu Banxia was speaking nonsense, to help her mentally relax.
However, Liu Yiqing and Xu Yino weren’t idle, their minds quickly turning, searching through their memories for what they studied. There seems to be some impression, but they couldn’t articulate it.
