In the fourth simulation, the entire reason Venerable Xuan had provided Ten-Thousand-Year Spiritual Milk and helped Zhou Qing push for the Nascent Soul Stage was so that they could explore that grotto-heaven shard together once he broke through.

During the sixth simulation, Zhou Qing had returned to Profound King Island after reaching the Deity Transformation Stage. At the time, Venerable Xuan had not yet passed away in meditation, so Zhou Qing conveniently subjected him to Soul Searching. It was then that he discovered the grotto-heaven shard and learned the whole truth.
